TaskFlow's platform team wired HashiCorp Vault's database secrets engine to a local PostgreSQL instance so apps can mint short-lived credentials on demand. The engine is enabled, the `taskflow-role` role is registered, and Vault itself is healthy — yet every `vault read database/creds/taskflow-role` call fails.
